Computerized Benefits Check-Up Program
Do you wonder whether you, your parents or your disabled family member might be eligible for benefit programs you don't even know about?
Shorefront JCC can help you find out if there are benefits you may be missing, and we can check your eligibility for 41 programs in one visit. We use sophisticated screening software to determine your/their eligibility for all of these entitlements and benefits.
You can come to our office, or if you are homebound we can come to you. A Shorefront JCC caseworker also provides this program regularly at Homecrest Community Services and Jay-Harama Senior Center. When you share with us the information listed below, we will determine whether you are eligible for any of the 41 programs. Then we will provide you with applications for all the programs for which you qualify. At our next meeting with you, we will help you complete the applications.
Help us complete your benefits check-up by having the following information with you:
- Proof of identity
- Proof of address
- Proof of family size
- Proof of income
- Proof of expenses-- rent, electric, gas, phone, medical, child care (if applicable) disability expenses (if applicable)
